Depending on your insurance plan, a referral or prior authorization may be required before certain appointments, imaging studies, procedures, or treatments. Before your appointment, please have your current insurance information, photo identification, referral or authorization information if applicable, relevant imaging and medical records, and contact information for your referring physician available.
Our team can help you understand what information may be needed before your visit. Because coverage, authorization requirements, deductibles, and other patient responsibilities vary by insurance plan, we recommend confirming your specific coverage and benefits directly with your insurance provider.

Please provide all required patient information and records, including demographics, insurance details, referral forms, clinical notes, imaging reports and available MRI/CT images. For urgent neurologic concerns, please call our office directly rather than relying solely on the online referral form.
